New patients to the clinic are required to complete a clinical intake form and sign a waiver. Please arrive no less than 30 minutes prior to your appointment to leave anough time to complete this forms. For your convenience, you can download the intake form here, print it out and complete it before you come to the clinic. In addition to this form, it would be helpful if you could also complete the form on metabolic typing, as well as a questionnaire to determine your Ayurvedic constitutional ('prakriti') type.
